Membership Terms

  • Three-month initial commitment with automatic renewal. Unused monthly treatment benefits expire at the end of each membership month.

  • Cancellations made with less than 24 hours' notice and missed appointments are subject to a $50 fee.

  • Members must complete a payment authorization form.
